A secondary endpoint is a prespecified outcome measure used to evaluate additional effects beyond a trial's primary endpoint.

Why secondary endpoints matter

Secondary endpoints can add information about:

  • additional efficacy dimensions;
  • duration of effect;
  • symptoms;
  • quality of life;
  • safety;
  • biomarkers; or
  • subgroup outcomes.

They can strengthen or complicate interpretation of the primary result.

Statistical caution

A favorable secondary endpoint does not automatically rescue a failed primary endpoint.

Investors should check whether secondary endpoints were prespecified, how multiplicity was handled, and whether the analysis is confirmatory or exploratory.
