Medical Loss Ratio (MLR): Health Insurer Benefit Costs vs Premiums

Medical loss ratio compares medical or benefit costs with premium revenue, but issuer medical-care and benefit-expense ratios can differ from statutory MLR definitions used for regulatory rebate tests.

Medical loss ratio (MLR) measures the share of premium revenue consumed by medical claims and related benefit costs on a stated basis.

A simplified operating form is:

If an insurer earns $10 billion of premium revenue and records $8.8 billion of medical costs, the simplified ratio is 88%.

A higher ratio usually means less underwriting margin

Holding other items constant, a higher medical-loss or benefit-cost ratio leaves less premium revenue available for administration, taxes, financing costs, and profit.

That makes the ratio one of the core underwriting indicators for managed-care companies. But the label and numerator are not perfectly standardized.

UnitedHealth Group reports a medical care ratio (MCR) calculated as medical costs divided by premium revenue. Humana and Elevance Health report benefit ratios or benefit expense ratios based on benefit expense relative to premium revenue. Those measures serve a similar analytical purpose, but investors should preserve each issuer's actual definition rather than automatically treating every ratio as identical.

Operating MLR is not automatically statutory MLR

The phrase "medical loss ratio" is also used in insurance regulation. Statutory or regulatory MLR calculations can include specified quality-improvement expenses, taxes, fees, credibility adjustments, and other rules that differ from a public company's operating medical-cost ratio.

Use the issuer's filing definition when analyzing earnings, and the applicable regulatory methodology when analyzing rebate or compliance requirements.

Medical cost trend is a major driver

If utilization, provider reimbursement, service intensity, drug costs, or coding intensity rise faster than premium pricing, the ratio can increase.

That is why Medical Cost Trend should be read beside MLR. Pricing can offset cost trend, but with a lag depending on the product and annual rate-setting cycle.

Member mix also matters. A company can add members while the ratio worsens if the new population has higher expected costs or if pricing does not fully reflect risk.

Prior-period claims development can move the ratio

Health insurers estimate claims incurred before all claims have been submitted and processed. When later experience differs from those estimates, Prior-Period Medical Claims Development can raise or lower current-period medical cost.

Favorable reserve development can temporarily reduce the reported ratio. Unfavorable development can increase it. Analysts should separate current-period underlying cost performance from revisions to prior-period estimates when the issuer provides enough detail.

Membership and premium yield matter together

Premium revenue broadly reflects both the number of covered members and the premium earned per member:

That makes Medical Membership and Premium per Member per Month natural companion metrics. Membership growth can increase premium revenue while also changing the medical-risk mix.

A simple example

Suppose a health plan earns $2.0 billion of premiums and records $1.78 billion of benefit expense:

If premium pricing rises 5% but medical cost trend runs 7% with no offsetting mix improvement, the ratio can deteriorate even while premium revenue grows.

Filing examples

UnitedHealth Group's second-quarter 2026 Form 10-Q states that MCR equals medical costs divided by premium revenue and discusses elevated medical cost trend and prior-period reserve development as key drivers.

Humana's second-quarter 2026 filing defines its benefit ratio as benefits expense divided by premiums revenue and discusses pricing, membership mix, medical and pharmacy trend, and prior-period development. Elevance Health similarly defines benefit expense ratio relative to premium revenue and discusses rate increases intended to recognize medical cost trends.

Medical loss ratio is an underwriting-cost ratio, not a universal standardized metric. Preserve issuer versus regulatory basis, product mix, premium denominator, benefit-cost numerator, medical-cost trend, prior-period reserve development, and membership mix before comparing health insurers.
