One reported claim does not always equal one prescription fill
Cigna counts non-specialty network prescriptions filled through 90-day programs and home-delivery prescriptions as three claims. Other network and specialty prescriptions count as one claim. That equivalency makes the metric useful within Cigna's own time series but prevents casual comparison with another PBM's prescription count.
Claim volume is also different from claims composition. Q2 volume declined even as Pharmacy Benefit Services adjusted revenues increased 8%, illustrating how mix can matter more than count.
