Evernorth Pharmacy Benefit Services adjusted income from operations is Cigna's issuer-defined pre-tax operating-profit measure for its Pharmacy Benefit Services operating segment.
The business reported $609M in Q2 2026, down 27% from $833M a year earlier even though adjusted revenues increased 8%.
Revenue growth did not translate into profit growth
Cigna attributed the quarterly profit decline primarily to client-focused initiatives, including large client contract renewals, and customer-focused initiatives. At the broader Evernorth level, the company said Pharmacy Benefit Services client and customer initiatives reduced pre-tax adjusted income growth by about 11 percentage points, with lower claim volume reducing it by another 2 points.
This is a non-GAAP segment measure. Read it with pharmacy claim volume and claims composition rather than assuming stronger drug-spend revenue automatically creates stronger PBM profit.
