A contract research backlog inclusion policy is the issuer's rule set for deciding which expected clinical-research revenue is credible enough to enter backlog and when it must be removed or revised.
Medpace recognizes new business awards after a signed contract or written pre-contract confirmation, but it can withhold amounts when funding, regulatory hurdles, or study timing remain too uncertain. It also excludes study amounts extending beyond three years from the measurement date. IQVIA takes a narrower contractual starting point in its backlog definition, describing future revenue from work not yet completed under signed contracts.
A signed opportunity can still fail the backlog test
The policy is meant to filter a commercial pipeline into a narrower operating measure. A sponsor may have agreed in principle on budget and scope while the CRO still judges future revenue too uncertain to include.
That makes the policy central to interpreting net new business awards and contract research backlog. Two companies can have similar commercial momentum but publish different backlog balances because their recognition windows and risk filters differ.
Cancellation rules can rewrite history
ICON changed its cancellations methodology effective October 1, 2025 so quarterly cancellation amounts include customer notifications plus studies that had become inactive or were identified as at risk. The change cut reported backlog by $3.9B at implementation, even though ICON said its treatment of awards entering backlog was unchanged.
This is why a backlog-policy change should be treated like a measurement-basis change, not merely a footnote. Investors comparing periods should identify whether the reported series has been recast and whether business cancellations are being recognized earlier.
